Forgotten Angels has a highly qualified direct care and administrative staff dedicated to providing excellent care and support of our clients.
OPEN – Program Director
Dan Mugubi – Program Manager
MA-Behavioral Science & Sociology, BA-Sociology
Adjunct Professor of Sociology-San Jacinto College
Former Sec. General of Samia South Welfare Organization, Kenya.
Previously, Mr. Mugubi has worked as a Quality Assurance Coordinator and Direct Care Professional for Forgotten Angels.
Kim Carter – Case Manager
Bachelor of Science-Psychology
Ms. Carter has been a case manager for people with special needs since 1993 and previously spent 5 years as a direct care professional.
CPR, PMAB, First Aid and Rape Crisis Counseling Certifications.
Michelle Forkner – Case Manager
Bachelor of Science-Psychology
Ms. Forkner has been a case manager since 2003.
CPR, First Aid and PMAB Certifications.
Christina Rivas – Case Manager
Bachelor of Science-Psychology
Since 2001, Ms. Rivas has worked as a service coordinator with general revenue in Texas Home Living (TxHml) and a case manager with another HCS Program before joining Forgotten Angels.
Previously worked for Mental Health and Mental Retardation Authority of Tarrant County.
Keithanie Singleton – Case Manager
Bachelor of Science-Sociology
Master of Science-Social Work
Before joining Forgotten Angels, Ms. Singelton previously spent 10 years working in the Correctional and Parole Division of the State of Texas.
Kate Wills – Case Manager
Bachelor of Arts-Psychology
Ms. Wills has worked in the field of human services since 1996 in the capacity of direct care staff, mental health aide, management, program specialist/supervisor, QMRP and case management.
CPR, First Aid and Behavior Modification Certifications.
Michelle Reescano – Day Habilitation Director
Since 1994, Ms. Reescano has worked with child and adults with special needs as direct care staff, as a program assistant and staff supervisor, a program manager, director of vocational services and as a case manager. She is also a certified Safe Behavior Management Instructor, is CPR, First Aid and PMAB certified and completed an Associate Bachelor’s Degree in Social Work in May 2006.
Cynthia Thompson – Community/Foster Care Coordinator
Associate Degree-Special Education
Ms. Thompson has 19 years experience working with people with special needs. She supervises Supported Home Living Staff and monitors services provided by Foster Care Parents. CPR, PMAB, First Aid Certifications
Stacie WOOSLEY – CASE MANAGER
Associates Degree - Applied Science
Previous Employee for Forgotten Angels for 6.5 years as a
DCS
CPR, First Aid, PMBA Certified, Mental Health Certificate in Case Management
CAN and currently studying for Registered nurse certification.
Nursing Department
- Sharon Gerlach – Registered Nurse, Master’s Degree in Counseling and National Board of Certified Counselors certified.
- Kaleen Weatherly – Licensed Vocational Nurse and currently studying for Registered Nurse certification.
